High School Athletics

 

Students who wish to participate on a Lynn Public Schools high school athletic team must register through FinalForms, our secure online athletic registration system.

FinalForms is used for all Lynn Public Schools high school athletic programs, including:

  • Lynn Classical High School
  • Lynn English High School
  • Lynn Vocational Technical Institute
  • Frederick Douglass Collegiate Academy (FDCA)
  • City Arts & Sciences Academy (CASA)
  • Harold Durgin Success Academy (HDSA)
  • Discovery Program (8th Grade students participating in approved high school athletic opportunities)

A parent or guardian must begin the registration process. High school students are also required to complete and sign their portion of the required forms before they are eligible to participate.

Please Note: High school registration requires two separate email addresses:

  • One parent/guardian email address
  • One student email address

Students will receive an email invitation from FinalForms after the parent/guardian has completed their portion of the registration process.

Important Registration Deadline

Students should be fully registered in FinalForms at least two weeks before the first day of tryouts or practice. This allows enough time for registration information and required documents to be reviewed and students to be cleared for participation.

FinalForms Allows Families To:

  • Complete and sign required athletic forms online.
  • Upload required documents.
  • Review and update student information at any time.
  • Save information from season to season and year to year.
  • Receive notifications when forms or documents need to be updated.
  • Easily register for future athletic seasons without re-entering information.

High School Athletic Requirements

Students must complete all required FinalForms registrations and signatures two weeks before participating in tryouts, practices, or competitions.

Depending on the requirements for their sport, families may be asked to:

  • Upload a current physical examination form (valid for 13 months).
  • Complete concussion and health-related acknowledgements.
  • Review athletic participation policies and expectations.
  • Complete transportation, media release, and sportsmanship agreements.
  • Complete all required parent/guardian and student signatures.

Students will not be cleared for participation until all required forms have been completed and approved.

Co-op Athletic Programs

Several athletic programs are offered as Lynn Public Schools co-op teams. Students attending Frederick Douglass Collegiate Academy (FDCA),  City Arts & Sciences Academy (CASA), and Harold Durgin Success Academy (HDSA), participate through their designated high school athletic placement.
