Middle School Athletics (Grades 6–8)

To participate in Lynn Public Schools middle school athletics, parents/guardians must complete the following requirements HERE: : Final Forms Registration

• Contact Information
  • Parent/guardian, emergency contact, and student information.
• Health History & Medical Profile
  • Medical history, allergies, and important health information.
• Medications
  • List any medications the student currently takes.
• Injuries & Hospitalizations
  • Report previous injuries, surgeries, or hospitalizations.
• Past & Ongoing Health Conditions
  • Identify any medical conditions that may affect participation.
• Emergency Medical Authorization
  • Permission for emergency medical treatment if needed.
• Physical Examination (Strongly Recommended)
  • While not required for middle school athletics, families are strongly encouraged to upload a current physical examination to ensure coaches and school nurses have the most up-to-date medical information.
• Head Injury/Concussion Reporting
  • Report any history of concussions or head injuries.
• Lynn Public Schools Concussion Policy
  • Review district concussion procedures and reporting requirements.
• Equipment & Uniform Responsibilities
  • Agreement to care for and return school-issued equipment.
• Transportation Acknowledgement
  • Review transportation expectations for practices and games.
• Media Release
  • Permission regarding photographs and videos of student-athletes.
• Athletic Sportsmanship
  • Review expectations for sportsmanship and positive behavior while representing Lynn Public Schools.

Important: Middle school registration must be started and completed by a parent or guardian. Only the parent/guardian signature is required.
