Registration & FinalForms

How do I register for athletics?

All Lynn Public Schools middle and high school athletic registration is completed through FinalForms, our secure online registration system: Final Forms Registration

. FinalForms allows families to complete required forms, upload documents, and update information online from any computer, tablet, or smartphone. Once your account is created, most information carries over from season to season and year to year.


When should I register?

Students should complete all registration requirements at least two (2) weeks before the first day of tryouts to allow enough time for athletic clearance.


Who starts the registration?

Middle School

  • Parent/guardian starts and completes the registration.
  • Only the parent/guardian signature is required.
  • Play Ball registration is through the schools designated Play Ball Coordinator, not Final Forms.

High School

  • Parent/guardian must begin the registration.
  • Both the parent/guardian and the student must complete and electronically sign the required forms.

Why does my high school student need their own email address?

High school registration requires:

  • One parent/guardian email address
  • One student email address

After the parent/guardian completes their portion, the student will automatically receive an email from FinalForms to complete their required forms.


Do I need to create a new account every season?

No. Your information remains in FinalForms. Before each sports season, simply log in, review your information, update anything that has changed, and submit your forms.


I forgot my password. What should I do?

Use the Forgot Password link on the FinalForms login page. If you still need assistance, click Get Help at the bottom of the FinalForms page.


Physicals & Medical Information

Do I need a sports physical?

High School

  • Yes. A current physical examination completed within the last 13 months is required before participating.

Middle School

  • A physical is not required, but it is strongly recommended to ensure coaches and school nurses have current medical information.

My physical expires during the season. What happens?

Students may not continue participating once their physical expires until a new physical has been submitted and approved.


Why do I have to complete concussion forms every year?

Massachusetts law requires annual concussion education for student-athletes and parents. These forms help ensure everyone understands concussion symptoms, reporting requirements, and return-to-play procedures.


Athletic Eligibility

What is the Lynn Public Schools "Two F's Rule"?

To remain eligible for athletics, student-athletes must maintain satisfactory academic progress.

A student-athlete who receives two (2) or more failing grades (F's) at the end of the applicable Marking Period becomes immediately ineligible once report cards are issued.

Eligibility is determined differently for each sports season.

Fall Sports

Eligibility is based on the previous school year's final grades (Y1 Grades).

Students must have fewer than two (2) F's under the Year 1 (Y1) grades.

Semester 1 (S1) grades are not used, even if they appear on the final report card.


Winter Sports

Eligibility is based on Marking Period 1 (MP1) grades.

If a student earns two (2) or more F's on the MP1 report card, they become immediately ineligible for the remainder of the winter season.

CASA students: Because MP1 report cards are issued after the winter season begins, CASA student-athletes follow the Fall Sports eligibility requirements until MP1 report cards are released. Once MP1 grades are available, the regular winter eligibility rules apply.


Spring Sports

Eligibility is based on Marking Period 2 (MP2) grades.

If a student earns two (2) or more F's on the MP2 report card, they become immediately ineligible for the remainder of the spring season.

CASA students: Spring student-athletes follow the Winter eligibility guidelines until MP2 report cards are issued. Once MP2 grades are available, the regular spring eligibility rules apply.


Does attendance matter?

Yes. Student-athletes must attend school according to Lynn Public Schools attendance policies in order to participate in practices or contests that day.


Does behavior affect athletic eligibility?

Yes. Student-athletes are expected to demonstrate good citizenship and follow all school rules. School disciplinary consequences may affect athletic participation.


Lynn Public Schools Transfer Policy

I transferred to another Lynn Public Schools high school. Can I still play sports?

Possibly.

Students who transfer from one Lynn Public Schools high school to another are ineligible to participate in any sport they played during the previous school year.

However, they may participate in sports they did not play during the previous or current school year.


What happens if I transfer during a sports season?

If a student transfers during a sports season:

  • They are not eligible to participate in that sport for the remainder of the current season.
  • They are also ineligible for that same sport during the following season.

When does the transfer policy begin?

The Lynn Public Schools Transfer Policy begins once a student enters Grade 9.

Any transfer between Lynn high schools during grades 9–12 may affect athletic eligibility.


Are there exceptions to the transfer policy?

Yes. Limited exceptions include:

  • Special education placement or therapeutic program changes
  • Acceptance into an application-based school that does not have its own athletic team
  • Homelessness or foster care placement
  • Transfers resulting from disciplinary action or district investigations
  • Court-ordered placement changes
  • Residency changes resulting in significant travel hardship
  • Admission from an application school waiting list
  • Programmatic changes (such as a school no longer offering a specific program)

To qualify for an exception:

  • Both principals and both athletic directors must agree in writing that an exception applies.
  • If any required administrator does not approve the exception, there is no appeal under the district policy.

I attend a Lynn high school that does not have athletic teams (CASA, FDCA, HDCA). Which school do I play for?

Students attending schools without athletic teams participate through their assigned home high school.

For athletics, the assigned home school is Lynn Classical High School or Lynn English High School, based on district assignment of where you live. LVTI is NOT an option.

What if I think my home school assignment is incorrect?

Please contact the Lynn Public Schools District Athletic Office before completing FinalForms registration so we can help ensure your student registers with the correct athletic program.


Seaforth Movement

What is the Seaforth Movement?

The Seaforth Movement is academic support for student-athletes.

High school student-athletes and parents  review and acknowledge this information during the registration process on FinalForms.


Equipment & Uniforms

What happens if equipment or uniforms are not returned?

Students are responsible for all school-issued uniforms and equipment.


Media Release

Why am I asked about photographs and videos?

Lynn Public Schools may use photographs or videos of student-athletes for:

  • School websites
  • District publications
  • Social media
  • Athletic promotions
  • School communications

Families may indicate their preferences through the Media Release form in FinalForms.
